I declare the above is true and complete to the best of my knowledge. I further give permission to Northside Community Resources to conduct fingerprint verification if deemed necessary. If I am offer volunteer placement with Northside Community Resources, I agree to abide by the policies set forth in the volunteer manual.

I understand that employees of Northside Community Resources (NCR) deal with a variety of confidential information. All NCR records and all proprietary information related to our employees, clients, orginization, and donors are confidential. No NCR information, including, but not limited to, documents, files, records, computer files or similar materials may not be removed from NCR premises without permission from the Executive Director. Confidential information reviewed by volunteer’s supervisor or the Executive Director.

I understand that I will not be an employee of NCR and will not receive pay or employee benefits from the agency. If I should, under some circumstances receive a stipend or reimbursement to cover expenses, this does not make me eligible for unemployment insurance benefits.

I assume all risks associated with providing direct services to clients of all programs through Northside Community Resources. This includes but is not limited to accidents, falls and contact with other individuals. Having read this waiver and knowing these facts and in consideration of this entry, I hereby for myself, parents, guardians, heirs, executors, and administrators waive any and all claims against sponsors and individuals associated with Northside Community Resources, their representatives and successors, and assign for any and all injuries suffered by the participation in programs.

The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent under applicable law.

I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN OF MY OWN FREE WILL.
