In September 2001, NCR (then the Rogers Park Community Council) launched the Senior Initiatives Program (SIP), which included a community wellness center, case management, computer instruction, and educational programming. In 2010, the program shifted to focus solely on at-risk and isolated seniors, and in July 2014, NCR absorbed another non-profit—Caring Connections for Seniors (CCS)—broadening its range of support and resources.

NCR is a delegate agency for the City of Chicago’s Department of Family and Support Services (DFSS). As such, we receive referrals from DFSS when neighbors, friends, or family members call 311 or 911 to alert the city of an at-risk senior and request assistance on their behalf. As a delegate agency, we respond to referrals within 24-48 hours. During which, we conduct an at-risk assessment, tailor a plan to get the seniors back on their feet, and connect them to additional resources.
